• Visa application form must be fully completed
• The form must be typed or written clearly in BLOCK LETTERS

• Size: 4 × 6 cm (passport-size)
• Background: White
• View: Frontal (face fully visible)
• Age of Photo: Must be taken within the last 3 months
• Attachment: Do not staple to the application form (loose attachment preferred)

NEW ZEALAND RESIDENCY STATUS

• Proof of your New Zealand residency or visa status must be provided
